2. SPONSOR:

The contest is sponsored by Curiva Medical Corporation dba Curiva Med Spa, located at 10300 Wilshire Blvd, Suite 200, Los Angeles, CA 90024 ("Sponsor").

3. ELIGIBILITY:

The contest is open only to legal residents of California who are 18 years of age or older at the time of entry. Employees, contractors, directors, and officers of the Sponsor, their immediate family members, and those living in the same household are not eligible to participate.

4. ENTRY PERIOD:

The contest begins on July 23, 2026 at 11:00 AM Pacific Time (PT) and ends on July 30, 2026 at 11:00 AM PT (the "Entry Period"). Entries submitted before or after the Entry Period will not be eligible.

5. HOW TO ENTER:

There are two ways to enter:

• Method 1 (Social Media): Follow the specific instructions outlined in the official contest publication (e.g., liking the post, following the account, tagging friends) during the Entry Period.
• Method 2 (Alternate Method of Entry - AMOE): To enter without using social media or taking promotional actions, you may submit an entry via email. Send an email to info@curivamedspa.com with the subject line "Onda PRO Giveaway Entry" and include your full name, phone number, and date of birth in the body of the email. Email entries must be received by the close of the Entry Period.
• Limit: One entry per person, regardless of the method of entry.

6. PRIZE, APPROXIMATE RETAIL VALUE (ARV), AND ODDS:

• Prize: Three (3) sessions of Onda PRO, limited to one (1) selected body area. 
• ARV: The total Approximate Retail Value ("ARV") of the prize is $3750 ($1250 per treatment area).
• Odds: The odds of winning depend entirely on the total number of eligible entries received during the Entry Period.

7. PRIZE REDEMPTION & MEDICAL CLEARANCE:

• Time Limit: The selected winner will have exactly 60 days from the date the prize is awarded to schedule and redeem their prize. If the prize is not redeemed within this timeframe, it will be forfeited.
• Medical Clearance: All aesthetic and wellness treatments are subject to standard medical clearance. The winner must complete a medical evaluation to determine if they are an appropriate candidate for the winning treatment. If the winner is deemed medically ineligible for the specific prize, the Sponsor will provide an equitable, alternative treatment for which the winner is medically cleared.
• Non-Transferable: Prizes are non-transferable, non-refundable, and cannot be exchanged for cash or combined with any other offers.

8. WINNER SELECTION & NOTIFICATION:

Winners will be selected in a random drawing from all eligible entries received on or about July 30, 2026. The winner will be notified via direct message or email within 48 hours of the drawing. If the winner does not respond within 48 hours of notification, the prize will be forfeited and an alternate winner may be selected.

9. TAXES:

All federal, state, and local taxes associated with the receipt or use of any prize are solely the responsibility of the winner. If the ARV of the prize is $600 or more, the winner will be required to provide their Social Security Number or Tax Identification Number to the Sponsor via a completed IRS Form W-9 before the prize can be awarded. An IRS Form 1099 will be issued in the winner’s name for the exact value of the prize.

10. PUBLICITY & MEDIA RELEASE:

By accepting the prize, the winner grants the Sponsor the right to use their name, social media handle, and likeness for promotional purposes across the Sponsor's marketing channels without further compensation. Furthermore, the winner agrees that the Sponsor may capture and use clinical "before and after" photography or videography of the treatment provided as part of the prize for use in future marketing materials.

11. PRIVACY & DATA USE:

Information collected from entrants is subject to the Sponsor’s Privacy Policy, which can be found at curivamedspa.com/policies/privacy-policy. By entering the contest, entrants agree to be added to the Sponsor's promotional email list and receive marketing communications. Entrants may opt-out of these communications at any time.

12. MEDICAL DISCLAIMER & PATIENT RELATIONSHIP:

Participation in this contest does not automatically establish a provider-patient relationship. Standard clinical protocols apply to all treatments. Any winner receiving medical or aesthetic services must consent to standard patient agreements prior to treatment. Results vary per patient and are not guaranteed.

13. PLATFORM RELEASE:

This promotion is in no way sponsored, endorsed, administered by, or associated with Instagram, Facebook, TikTok, or any other social media platform. By entering, entrants completely release the respective social media platforms from any liability.

14. RIGHT TO MODIFY:

The Sponsor reserves the right to cancel, modify, or suspend the contest at any time in the event of fraud, technical failures, or any other factor beyond the Sponsor's reasonable control that impairs the integrity of the promotion.
